Introduction

Life IV Therapy PLLC, doing business as Life IV Weight Loss (collectively, “Life IV,” “we,” “us,” or “our”), is a healthcare provider that owns and operates www.lifeivtherapy.com and www.lifeivweightloss.com (collectively, the “Websites”). This Privacy Policy explains how we collect, use, disclose, and protect personal information when you visit our Websites, contact us, create an account, use our online portal, schedule an appointment, complete forms, make a payment, or otherwise interact with our services.

Life IV currently provides clinical services in Michigan and Ohio. We do not currently offer a mobile application. If we introduce one or materially change our privacy practices, we will update this Privacy Policy as appropriate.

This Privacy Policy applies to personal information handled through our Websites and related administrative services. Our use and disclosure of protected health information (“PHI”) are also governed by the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”), other applicable laws, and our Notice of Privacy Practices.

HIPAA and Our Notice of Privacy Practices

Life IV is a healthcare provider and a HIPAA covered entity. Information that identifies you and relates to your health, healthcare, or payment for healthcare may be PHI when it is created, received, maintained, or transmitted by Life IV in its capacity as a healthcare provider.

Our Notice of Privacy Practices explains how we may use and disclose PHI, describes your rights concerning PHI, and explains how to exercise those rights. We provide it to patients during their initial paperwork process and annually thereafter. You may also request a copy from our Privacy Officer using the contact information below.

If this Privacy Policy conflicts with our Notice of Privacy Practices regarding PHI, the Notice of Privacy Practices and applicable law will control.

Information We Collect

The information we collect depends on how you interact with us.

Information you provide. We may collect:

  • Identifiers and contact information, including your name, date of birth, address, email address, and telephone number;
  • Account and portal credentials;
  • Appointment and service information;
  • Health and medical information submitted through intake forms, communications, the patient portal, or during care;
  • Insurance, eligibility, claims, prior-authorization, and billing information;
  • Payment and transaction information;
  • Documents, photographs, messages, and other information you choose to submit; and
  • Marketing and communication preferences.

We do not ordinarily collect Social Security numbers, passport information, biometric identifiers, precise geolocation, or information about sexual orientation through our public Websites. If information of this nature is clinically necessary and collected in connection with healthcare, we handle it according to HIPAA, our Notice of Privacy Practices, and other applicable law.

Information collected automatically. When you visit our public Websites, we and our service providers may automatically collect your Internet Protocol address; browser, device, and operating-system information; approximate location derived from your IP address; referring website or advertisement; pages viewed; links selected; visit date and time; cookie identifiers; and similar online activity information.

Information from other sources. We may receive information from healthcare providers, health information exchanges, laboratories, insurers, prior-authorization services, payment processors, and other entities involved in your care or our operations, as permitted by law.

How We Use Information

We may use personal information to:

  • Provide, coordinate, and manage healthcare services;
  • Create and maintain patient records;
  • Review records, laboratory results, and other information relevant to treatment;
  • Communicate about appointments, treatment, prescriptions, laboratory services, insurance, billing, and administration;
  • Verify insurance benefits, submit claims, obtain prior authorizations, pursue medication coverage or assistance, and collect payment;
  • Operate patient accounts, portals, scheduling, intake, electronic forms, telehealth, and payment functions;
  • Respond to inquiries and provide support;
  • Send newsletters and promotional communications when permitted by law;
  • Operate, secure, troubleshoot, and improve our Websites and services;
  • Conduct internal quality-improvement and operational analyses as permitted by law, including with aggregated or appropriately de-identified information when suitable;
  • Detect and prevent fraud, misuse, or security incidents; and
  • Comply with legal obligations and protect the rights, safety, and property of our patients, workforce, organization, and others.

We do not sell patient information or PHI. We do not upload patient lists to advertising platforms or use PHI for targeted advertising.

How We Disclose Information

We may disclose information as described below and as otherwise permitted or required by law. When information is PHI, we use and disclose it according to HIPAA and our Notice of Privacy Practices.

Healthcare treatment and coordination. We may exchange information with healthcare professionals and organizations involved in your treatment. Life IV accesses ProMedica Health System’s Epic electronic health record through its community portal to review patient records, results, and other information relevant to treatment and to upload documents to a patient’s Epic chart when appropriate.

Laboratory services. We may disclose orders, demographic information, insurance information, and other information needed by Quest Diagnostics and other laboratories used in patient care, and we may receive laboratory results from them.

Insurance, billing, and payment. We may exchange information with health insurers and other payers to determine eligibility and benefits, submit and process claims, obtain payment, respond to audits, and perform related healthcare operations. Online payment functions are provided through PracticeQ/IntakeQ and its payment services. We do not directly store complete payment-card numbers when a payment processor handles the transaction.

Prior authorizations and medication access. We may disclose information to Forus, CoverMyMeds, insurance carriers, healthcare providers, and other appropriate organizations to submit and manage prior authorizations, investigate benefits, pursue appeals, coordinate medication coverage, and evaluate medication-support options.

Service providers and business associates. We use organizations that support our clinical, administrative, technical, and business operations, including:

  • Epic Systems and ProMedica Health System for electronic health record access and exchange;
  • PracticeQ/IntakeQ for the patient portal, scheduling, intake, electronic forms, telehealth, and online payments;
  • ActiveCampaign for permitted email communications and marketing;
  • Google Voice for telephone and text communications;
  • SRFax for fax communications;
  • Hostinger for website hosting;
  • Elfsight for public website contact forms;
  • QuickBooks for accounting; and
  • Other vendors providing security, information technology, compliance, legal, or administrative support.

When HIPAA requires it, we enter into business associate agreements requiring organizations that receive PHI on our behalf to safeguard it appropriately.

We do not submit PHI through ActiveCampaign, advertising platforms, or public website analytics tools. Please do not include medical details or other sensitive information in a public website contact form. Use the patient portal or contact our office to communicate about your care.

Legal, safety, and organizational disclosures. We may disclose information when reasonably necessary to comply with law or valid legal process; respond to authorized regulatory requests; prevent fraud or security threats; protect health or safety; enforce agreements; or protect rights or property. We may also disclose information in connection with a merger, acquisition, financing, reorganization, or transfer of business assets, subject to applicable privacy and healthcare laws.

Cookies and Website Analytics

Our public Websites use cookies, pixels, tags, and similar technologies. Cookies are small files stored on your device that allow a website to recognize a browser, remember preferences, measure activity, and support advertising.

We use Google Analytics, Google Ads, Meta advertising technologies, and Google Tag Manager on public-facing portions of our Websites. These services may collect IP addresses, cookie identifiers, device and browser information, referring information, pages viewed, links selected, and similar website activity. We use this information to understand Website performance, measure general advertising effectiveness, and improve our public Websites.

We do not intentionally place these advertising or analytics technologies within authenticated patient portals, intake forms, appointment-scheduling workflows, online payment pages, or pages used to communicate PHI. We do not provide PHI or patient lists to Google, Meta, or other advertising platforms for targeted advertising.

Our Websites provide a cookie-consent banner. You may use the choices offered through that banner and your browser settings to accept, reject, or delete certain cookies. Blocking cookies may affect some Website features. Browser-based choices generally apply only to the browser and device on which they are made.

Our Websites do not currently offer a separate “Do Not Sell or Share” link, privacy preference center, or automated Global Privacy Control response. Because we do not sell personal information, you may contact our Privacy Officer with questions or requests about our data practices.

Email, Telephone, and Text Communications

We may contact you by email, telephone, or text for healthcare, appointment, billing, customer-service, and administrative purposes as permitted by law and based on the contact information and permissions you provide.

If you opt in to promotional messages, you may withdraw permission by following the unsubscribe instructions or contacting us. Opting out of promotions will not prevent necessary treatment, appointment, billing, safety, or other non-promotional communications.

We do not sell mobile telephone information or share text-message opt-in data or consent with third parties for their independent marketing. Electronic communications may carry privacy risks. Please use the patient portal when available for detailed or sensitive healthcare communications.

Children and Minors

Our public Websites are not directed to children under 13, and we do not knowingly solicit personal information from children through our public Websites without appropriate involvement from a parent or legal guardian.

Life IV may provide healthcare to a minor when clinically appropriate and when necessary consent from a parent, legal guardian, or other legally authorized person has been obtained, except where applicable law allows or requires the minor to consent independently. We handle minors’ information according to HIPAA, applicable state law, and our Notice of Privacy Practices.

If you believe a child submitted personal information through a public Website without appropriate permission, contact our Privacy Officer. Medical-record information will not be deleted when retention is required or permitted by law.

Data Security

We maintain reasonable administrative, technical, and physical safeguards designed to protect personal information and PHI against unauthorized access, use, alteration, disclosure, or destruction. No method of electronic transmission or storage is completely secure, and we cannot guarantee absolute security.

You are responsible for protecting your portal credentials and should not share your password. Notify us promptly if you believe your account or information has been compromised.

Data Retention

We retain information according to our established retention schedules and for as long as reasonably necessary to provide services, maintain medical and business records, complete transactions, comply with legal requirements, resolve disputes, enforce agreements, and protect our rights. Periods vary with the nature of the information and applicable healthcare, insurance, accounting, and legal requirements.

Your Choices and Rights

You may update certain account or contact information through the patient portal or by contacting us; unsubscribe from promotional email using the link in the message; manage certain cookies through our cookie banner or browser settings; and contact us with questions or requests concerning personal information.

Your rights concerning PHI—including rights to request access, amendment, restrictions, confidential communications, and an accounting of certain disclosures—are described in our Notice of Privacy Practices. Those rights may be subject to limitations and verification requirements under applicable law.

Third-Party Websites and Services

Our Websites may link to patient portals, social media platforms, or other third-party services. When you follow a link, that third party’s privacy policy generally governs information you provide directly to it. This Privacy Policy does not control independent third-party privacy practices.

Changes to This Privacy Policy

We may update this Privacy Policy to reflect changes in our services, vendors, data practices, or legal obligations. We will post the revised policy on our Websites and change the “Last Updated” date. We will provide additional notice or obtain consent when required by law.
